To get started we need your Customer Information. This will allow us to provide you the highest level of customer service. Please complete a Customer Information Form and "Submit" to send it to us.

Sample submission must be on our Sample Submission Form. Please provide Purchase Order Number or other tracking number for us to reference on the invoice. Complete the form and click "Submit." After you submit your form, you can click on the Print icon. This will print one copy for you to include with shipment of samples. Please sign this copy of the form to authorize testing. This will also email a form to us so we know samples will be coming. Be certain to note any special cultural conditions required so the proper arrangements can be made.

 

Sample Submissions:
Pure cultures, slants preferred, parafilmed

Agar plates will be accepted, parafilmed

Ship samples in accordance with Biohazard shipping regulations

Ship overnight to:

          Attn: Quality Control Microbiology

          Biological Process Development Facility

          Lincoln, NE 68588-0668

Results will be reported as directed on Customer Information Form.

Pricing:
1 sample $360
2-4 samples $250 each
5 samples or more $150 each

Invoicing will be done upon completion of testing as directed on Customer Information Form.

Payment terms are 30 days.
